Сonsent to process personal data

I hereby freely, voluntarily and in my own interest give my consent in order to fulfill the requirements of the Regulation on Personal Data Processing, Federal Law No. 152-ФЗ and within the framework of acceptance of the offer, located in the information and telecommunications network of Internet at the following link https://rizzle.ru/page/oferta, to Individual entrepreneur A.M. Nazarova, INN 773377841415 OGRNIP: 319774600405196 (hereinafter "Operator") to:

• to process my personal data using automation means (and without it), namely: collection, recording, systematization, accumulation, storage, clarification (updating, change), extraction, use, transfer (distribution, provision, access, including placement/publication on the corporate portal/website and in information systems of the Operator with limited access to personal data of Operator employees only), depersonalize, block, delete, destruct my personal data, such as: surname, name, middle name, identity document data, work position, place and date of birth, phone number and e-mail, photograph and other data specified in the Regulation; 
• to obtain my personal data from third parties (other sources containing my personal data); 
• to transfer my personal data to counterparties and third parties for the purpose of execution of contracts concluded by the Operator.

I authorize processing of my personal data for any purposes not prohibited by the legislation of the Russian Federation, including ensuring compliance with laws and other regulations; conclusion and regulation of labor relations and other directly related relations; reflection of information in personnel documents; salary accrual; providing information to a credit organization for bank card registration and transfer salary to this card; ensuring my safety; monitoring quantity and quality of work performed by me etc. 

I pledge not to disclose to third parties and not to disseminate personal information about the subjects of personal data, to which I will get/can get access within the frame of cooperation with the Operator.

I confirm that the Operator has explained to me the legal consequences of refusal to provide my personal data, liability measures for violation of legislation in the field of personal data protection, as well as my rights and obligations as a subject of personal data arising in connection with processing of my personal data by the Operator, including the possibility to withdraw my consent at any time by applying to the Operator with the appropriate application.

This consent shall take effect from the date of its signing until it is revoked in writing.
